The UCC Financing Statement (UCC1) form is filed by a creditor to give notice that it has or may have an interest in the personal property of a debtor (person who owes a debt to the creditor as typically specified in the agreement creating the debt). This form is filed in order to "perfect" a creditor's security interest by giving public notice that there is a right to take possession of and sell certain assets for repayment of a specific debt with a certain priority.
The UCC Financing Statement Addendum form (UCC1AD) is used to list long debtor or secured party names that do not fit on the UCC Financing Statement (UCC 1) form and to add additional collateral descriptions, real estate record information and any miscellaneous information. This form should be submitted as an attachment to the UCC Financing Statement form.
The UCC Financing Statement Additional Party form (UCC1AP) form can be used in multiples to continue adding additional Debtor or Secured Party names as needed. This form should be submitted as an attachment to the UCC Financing Statement form (UCC1).
The UCC Financing Statement Amendment form (UCC3) is used to file a termination, assignment, continuation, party information and collateral changes that affect a UCC Financing Statement.
The UCC Financing Statement Amendment Addendum (UCC3Ad) is used to add additional collateral, real estate record information and miscellaneous information not provided on the Financing Statement Amendment form. This filing should be submitted as an attachment to the UCC Financing Statement Amendment form (UCC3).
The UCC Financing Statement Amendment Additional Party form (UCC3AP) is used to add additional debtor and secured party’s and miscellaneous information not provided on the UCC Financing Statement Amendment Form (UCC3). This form should be submitted as an attachment to a UCC Financing Statement Amendment form.
A person may file an Information Statement (UCC5) on a UCC Financing Statement to which the Information Statement relates if they believe a record is inaccurate, wrongfully filed or filed by a person not entitled to do so. This filing is not an amendment, no changes will be made to the record as a result of this type of filing.
The International Association of Commercial Administrators (IACA) forms are the only acceptable forms for filing UCC financing statements.
